Why Should Care Team Be Your Home Care Agency of Choice?

For 30-plus years, since 1988, the Stoker Family has owned and operated Care Team Home Care. For 30-plus years our St. Petersburg Care Team has provided in-home skilled nursing and caregiver services including: in-home eldercare, veterans care, wound care, skilled and non-skilled nursing services, post-surgery or post-hospitalization homecare, and specialty infusions.

Our long history and years of experience in the home healthcare industry assures our clients and patients of the most professional, quality healthcare, no matter the intricacies of care, the length of time, or circumstances.  Our highly skilled team of nursing and caregiving professionals, and our experienced management team understand that this is a difficult time for both client and patient. That’s why we are on hand for consultation, evaluation, health history, and home environment reviews, medical team alliance, and prescription reviews, so that our clients and patients can make informed, yet sometimes difficult or life-changing decisions.

Skilled Nursing Visits

Skilled nursing care is provided by trained registered nurses under the supervision and guidance of a medical doctor. Basically, it is the same level of nursing care you would get as a patient in the hospital or a rehab facility. Skilled nurses can provide some physical therapy, wound care, intravenous injections, catheter care, and more. They are responsible for aiding in your continuing recovery after an illness, injury, or surgery.

And our skilled nursing staff will offer you a complete patient assessment.  They’ll check your living space for potential risks of tripping or falling. They will review your medical history, make a list of medications, and answer any questions and address any concerns you may have. They will even give you options for home safety devices to add to your sense of security and well-being.

Specialty Infusions

In addition to skilled nursing visits, the St. Pete Care Team specializes in infusion therapy.  Our team is trained to administer medication via IV, directly into the bloodstream, or intramuscularly via a needle.  These outpatient infusion procedures are used to treat a variety of medical conditions and infections.  They are safe, relatively painless, and can offer our patients almost immediate relief.

Most people can benefit from Specialty Infusions.  Patients with chronic pain, patients experiencing nausea from chemo treatments or other medical ailments, where infusion therapy is needed.  Even those individuals seeking infusion therapy to bolster their physical training regimen or to increase their energy or improve their lifestyle.  Whatever the reason, we are here to help.

Quality Care You Can Trust

Care Team Home Care’s focus is on quality care with a hands-on team approach.  It consists first of sitting with the patient or the patient’s caregivers to do a thorough assessment of the patient’s health or recovery status to determine exactly what the patient’s needs are.  What type of services are needed? Are they ambulatory? Do they need medical supervision? Do they need toileting assistance? Nursing-care or non-medical caregiving?  Short term or long-term care? These are only a few of the questions we determine answers to during our evaluation.

Once we have the detailed answers, we devise a Plan of Care, we then discuss the patient’s needs with our Care Team’s Service Coordinators to assign the appropriately qualified staff members to the case.

But our services don’t end there.  Once our personnel are scheduled and we have informed the patient, the caregiver will then come out and provide a brief summary of each individual involved in his or her care. Patient, client and nurse or aide are then introduced to each other, expectations are outlined, questions are answered, routines are explained, and our services begin. When providing services, our caregivers, nurses, and aides follow a regimented discipline ensuring safety and hygiene, so you can feel confident that we are keeping your loved one safe.

All Care Team Home Care caregivers document the services they provide based on the patient’s Plan of Care. These documents are reviewed by a Care Team Registered Nurse for adherence to the care plan, as well as to ensure that any changes or notations are ascertained.

Besides a regular review of notes, we assign a Registered Nurse to visit the patient’s place of residence on at least a quarterly basis to monitor our services and our caregivers, to ensure everything and everyone is up to our high standards and to evaluate and address any concerns or changes that might be requested or needed.
