Skilled Nursing Visits

Skilled nursing care is provided by trained registered nurses under the supervision and guidance of a medical doctor. Basically, it is the same level of nursing care you would get as a patient in the hospital or a rehab facility. Skilled nurses can provide some physical therapy, wound care, intravenous injections, catheter care, and more. They are responsible for aiding in your continuing recovery after an illness, injury, or surgery.

In addition to skilled nursing visits, the Tarpon Springs Care Team specializes in infusion therapy.  Our team is trained to administer medication via IV, directly into the bloodstream, or intramuscularly via a needle.  These outpatient infusion procedures are used to treat a variety of medical conditions and infections.  They are safe, relatively painless, and can offer our patients almost immediate relief.

Quality Care You Can Trust

Our team approach is unique in the industry. It consists first of sitting down with the patient or the patient’s caregivers to do a complete assessment of the patient’s health or recovery status to determine exactly what the patient’s needs are.  What type of services are required? Is the patient ambulatory? Do they need medical supervision? Nursing care or non-medical caregiving?  Toileting assistance?  Short-term or long-term care? These are only a few of the questions we determine answers to during our evaluation.

Once we have the detailed answers, we devise a Plan of Care, then we discuss the patient’s needs with our Care Team’s Service Coordinators so that they can assign the appropriately qualified staff members to the case.  

But our services don’t end there.  Once our Lakeland Care Team personnel are scheduled, we inform the patient that we are ready to begin their care. The caregiver or aide will then provide a brief background of each individual involved in your care. Patient, client and nurse or aide are then introduced to each other, expectations are outlined, questions are answered, routines are explained, and our services begin. When providing services, our caregivers, nurses, and aides follow a regimented discipline ensuring safety and hygiene, so you can feel confident that we are keeping your loved one safe.

All Care Team Home Care caregivers document the services they provide based on the patient’s Plan of Care. These documents are reviewed by a Care Team Registered Nurse for adherence to the care plan, as well as to ensure that any changes or notations are quickly ascertained. 

Besides a regular review of notes, we assign a Registered Nurse to visit the patient’s place of residence on at least a quarterly basis to monitor our services and our caregivers, to ensure everything and everyone is up to our high standards and to evaluate and address any concerns or changes that might be requested or needed.

Safety is First and Foremost

When providing services, our skilled nurses and aides follow a regimented, well-ordered discipline ensuring safety and hygiene, so you can feel confident that we are keeping your loved one safe from infections or contamination.  We wash our hands, we wear masks, we clean and disinfect surfaces, medical equipment, and devices. 

All Care Team Home Care caregivers, nurses, and aides meticulously document the services they provide based on the patient’s Plan of Care. These documents are reviewed by a supervising Care Team Home Care Registered Nurse for adherence to the Plan, as well as to ensure that any changes or new entries are noted.
